Enrollment

  • Student is over the age of 5 (student must be 5 by August 1st), but under 22.
  • A Legal Ohio Resident
    • We will require one of the following items showing the parents/guardians name and address:
      • Utility Bill (Gas, Electric, or Water)
      • A full copy of lease or mortgage papers.
      • Voter Registration Card
      • Most Recent Bank Statement
      • Most Recent Rent Receipt
      • Please contact the QDA office if you are living temporarily with someone. There's an alternative document to sign.
    • Documentation of the student's residency shall be a good faith effort to accurately identify the correct residence of the student. The items must be current and include a street address; a P.O. Box cannot be used to validate residency records.
  • Internet Access: QDA does not provide an Internet connection. It is the parent/guardian's responsibility to obtain reliable Internet access.

You will get an email (if you didn't get an email check your SPAM or Junk mail folder) with your account credentials and another email stating that we received your application. Additional emails will be delivered as your application is process. You may check the status of your application by logging into Maestro and selecting View Application.

Here is what each status update means:


Step 1
Submitted - Your application has not been reviewed or we have not received the enrollment packet. [email sent]

Step 2
New Phila - The New Phila office is requesting records and other documentation.
Berlin - The Berlin office is requesting records and other documentation.
East Liverpool - The East Liverpool office is requesting records and other documentation.
Steubenville - The Steubenville office is requesting records and other documentation.
Pending Review - Special Services - The Special Services department will be requesting records and other documentation.

Step 3
Pending Review - A final check to ensure all paperwork is submitted is being done.

Step 4
Processed -We have all of the required paperwork to begin creating a course schedule. [email sent]

Step 5 - High School Only
Complete Credit Check - A guidance counselor will review academic history to determine courses and credits needed.

Step 6
Schedule Courses - A course schedule will be generate (please note, course schedules can be changed upon request)

Step 7
Assign IS - An Instructional Supervisor will be assigned. [email sent]

Step 8
Schedule Orientation
Your Instructional Supervisor will contact you to schedule an orientation.

Once the orientation is completed the student will be enrolled.

Applications can only be processed once we have received academic records. If you wish, you may request academic records (official or unofficial) from your last school and provide them to QDA. A QDA staff member will let you know what other outstanding documentation must be submitted. Again, we can't process applications without academic records.

There are two applications available for prospective students. If a student is under 18, their parent or legal guardian must create a guardian account first. Students that are 18 or over can self apply.

-Application for students under 18.

-Application for students 18 and over.

Once you have submitted the application you will receive two emails. The first email will have your account information so you can sign back in. The second email will confirm that we have received your application. If you do not receive an email from the MaestroSIS system, please check your SPAM or JUNK mail folder. If you still haven't received it, please call us at 1-866-968-7032 or email us at info@sis.qda.education

QDA will require an updated application for you to be considered for re-enrollment. Please contact the QDA office to obtain a re-enrollment packet.

To be considered for enrollment, QDA must receive a student application, proof of Ohio residency, and an enrollment packet. An application will be considered incomplete until all items are received.


Proof of Ohio Residency

Applicants can update their Proof of Residency to the Maestro Student Information System during the applications process or email it to records@go2qda.org. The documents should show the entire page, company name/logo, and your mailing address. You may black out any amount owed; however, we can't accept a disconnect notice.

Please contact the QDA office if you are living temporarily with someone. There's an alternative document to sign.

Enrollment Packet

After an application is submitted, the system will email a link to the enrollment packet. If you did not receivedthe email, please check your SPAM or Junk folder. The 2019-20 Enrollment Packet can also be found here.
